About this template
A ticket that says only "it is broken" costs two more emails before anyone can start. This asks in three short steps: who you are, what the ticket is about, and what actually happened.
Three steps
- Your details - name, email and a phone number.
- The ticket - department, priority, subject and category.
- Detail - the description, screenshots and a consent box for account access.
The progress bar shows how much is left, which is what stops people abandoning a long form at the first screen.
