I give permission for my child to take part in the activities run during the camp. I understand that sport carries a risk of injury, and I accept the ordinary risks that come with taking part, travelling to and from sessions, and using the facilities.

Medical treatment

If my child is injured or becomes unwell and I cannot be reached, I authorise a qualified medical professional to give whatever treatment they judge necessary, including examination, x-ray and minor procedures. Camp staff may give first aid before a medical professional arrives.

Every reasonable effort will be made to contact me first. This permission applies only for the dates of the camp my child is registered for.
